Getting To Know You

Our team receive your enquiry and discuss your specific care requirements with you and get to know your personality and preferences. We will provide information on the services that are available to you, as well as an outline of costs. Our Services are offered UK Nationwide, no location is out of reach for us.​

Care Team Assessment

We will arrange for a member of our Care Team to visit you and carry out a comprehensive care assessment of your care needs, including a risk assessment to ensure your safety. Following this visit, a fully costed care package will be offered.​

Creating Your Plan

We will finalise your care plan with you and your family, making sure you are happy with every aspect.​

Selecting Your Career

We will identify suitable healthcare professionals to support you in-line with your care plan and arrange for you to meet them to ensure that all of your needs are met. If any additional training is required to support your care requirements, this will also be coordinated.​

Founded From Personal Experience

Having worked in the home care industry for many years a group of nurse entrepreneurs saw an opportunity to make the home care world a better place. As intensive care nurses they understood the immense struggles of looking after clients withcomplex careneeds.

They came to appreciate that it was much more than performing clinical nursing care duties, it was also about empowering people to be independent and allowing them to take positive risks. They started Four Oaks Healthcare to go beyond the normal home care duties.
